If you read Anthony's autobiography Scar Tissue, he explains most of the songs from the Blood Sugar Sex Magik album. This one is focused on his drug use after Hillel Slovak died from a cocaine and heroin speedball. Anthony felt responsible for he was the one who introduced Hillel into heroin. After Hillel's passing, Anthony ironically dives deeper into drug use. This song is sort of his expression of angst and turmoil he feels not only towards himself but also for the loss of Hillel.
