Venezuela is the world's No. 5 oil exporter. All that oil moves through terminals on Lake Maracaibo -- the largest lake in South America -- and then through a brackish channel to the sea.
On November 6 2005, two vessels are intending to pass one another during daylight hours, in the calm waters of Lake Maracaibo. They are the LNG carrier M/T Maersk Holyhead and bulk carrier M/V Pequot -- large, modern vessels with the latest communications & navigational equipment.

Actually, this is the collision between the Maersk Hollyhead (sister ship of my previous vessel Maersk Humber) and a Bulk carrier. It is a common myth that it was steering gear failure, what actually happened is the vessel was navigating in a narrow channel, I know it looks like open water but remember there is alot of ship underwater too and the vessel only had a narrow channel of water at sufficient depth in which to navigate, what happened is something called
interaction.

where do you get this info? The accident was due to a bank effect causing the mearsk ship to get of course and crashing into the meeting vessel. Bouth vessels are constrained by draft, and underway.

rudder could fail or the engines it takes these ships a long way to change course or just pilot error
